When you are a patient at the clinic, it is necessary for us to register and use certain information about you. We are obliged to do so according to the Authorization Act, the Executive Order on Record Keeping and the Health Act.
The information that we register and process about you must be stored securely, and we have a duty of confidentiality. Disclosure of information about your health conditions can, where necessary, be exchanged internally at the clinic,
, whereas disclosure of health information to persons etc. outside the clinic can generally only take place with your consent, cf. Chapter 9 of the Health Act.
We register and use certain information about you for billing purposes if it is necessary to be able to make a payment for the clinic's treatment of you, for example via your region, municipality or an insurance company. The clinic is obliged to keep your medical records for 10 years after your last contact with the clinic. If you choose to change dentist, we will transfer your records to your new dentist, who will then take over the obligation to store your records. In special cases, as described in the Executive Order on Record Keeping, we may keep your records for a longer period of time than described above.
Information about you that the clinic stores for billing purposes is stored for as long as it is relevant and required for the clinic's billing and accounting purposes. Information about you for billing purposes is registered on the basis of Article 6(1)(b) and (f) (until May 25, 2018, the information is registered on the basis of Section 6(1)(2) and (7) of the Personal Data Act).
You are at all times entitled to access the information that the clinic registers and processes about you. If you believe that we have registered incorrect information about you, you can ask us to correct the information. We are not allowed to delete information in your medical record, but if you think there is an error in the record, you can ask us to add your comments.
If you want access to documents, you must send a written request to info@plus1.dk. You may be asked to document that you are who you claim to be. Should you wish to complain about the clinic's processing of your personal data, you can complain to the Danish Data Protection Agency, which you can find more information about at www.datatilsynet.dk.
It is the Danish Patient Safety Authority that supervises the rules in the health legislation. You can find more information about the Danish Patient Safety Authority at www.stps.dk. When you book an appointment on our website, we ask you for your name, phone number and email. We use this information to confirm your treatment, including providing you with any information about our treatments and services that you have requested. If you specifically ask us not to contact you in the future, we will keep a copy of your request to record your request and honor your request not to be contacted.
We use external data processors and data controllers who have access to personal data from our website and in our systems in order to fulfill the purposes, which include accounting and backup.
The data processors handle personal data according to our instructions and only for the purposes we have mentioned here in the privacy policy. We have imposed various security requirements on the companies to ensure that personal data is treated confidentially and securely. Other data controllers only receive personal data from us on a lawful basis and are independently obligated to comply with applicable laws.
As a general rule, we do not transfer your personal data to countries outside the EU/EEA. If this should become necessary, it will be done in accordance with legislation, including ensuring the necessary basis for secure transfer.
We will regularly review this Privacy Policy and update it from time to time as our services and use of personal data evolve. If we wish to make use of your personal data in a way we have not previously identified, we will contact you to provide information about this and, if necessary, to ask for your consent.
We update the version number and date of this document every time it changes.
