Privacy Policy for PLUS1

Dear Patient,

This is PLUS1’s Privacy policy while using our website and for patient treatment. This states that we, PLUS1, take full responsibility for your personal data, and that we are obliged to inform you of your rights, and how we handle your personal information.

Retrieving and processing this personal data is necessary when you are a patient at one of PLUS1’s clinics. We have an obligation, according to the authorization act, the record-keeping order, and the health act to store your personal data that we collect and administer for you securely. We must abide by these confidentiality laws. When necessary, the disclosure of your data regarding health-related issues can be used, for internal use only. According to the health act chapter 9, the disclosure of your health-related information to persons outside our clinic will only occur with your consent.

We register and use certain information about you for billing purposes. This data is necessary to create an invoice for the clinic’s treatment of you as a patient, e.g., via your region, municipality, or an insurance company. The clinic is obliged to keep your medical records for 10 years after your last visit. If you choose to change dentists, we will transfer your records to your new dentist, who will then take the responsibility and keep your records. Under special circumstances, which are described in the Record-Keeping Order, we are allowed to keep your data longer than described above.

We store the information collected for billing purposes for as long as it is relevant and required by law. The information about you, for invoicing purposes, are registered based on “art. 6, stk. 1, litra b og f (until 25. May 2018 registering information based on personal data act § 6, stk. 1, nr. 2 og nr.7).

At any given time you can retrieve access to the information we have registered and processed for you. If you feel that we have registered the information incorrectly, you can request that we correct it. We are unable to remove any information in your journal. If you feel that we made a mistake, you will always be able to include your remarks.

To gain access to your journal, you must send a written request to info@plus1.dk. You will be required to document, that you are the person you claim to be. If you want to make a complaint regarding the clinics’ processing of your personal data, you can send a complaint to Data protection Authority. It is the Agency of Patient Safety, which supervise these rules in health legislation. You can find more information about the Agency for Patient Safety at www.stps.dk.

When using our website, we collect and use certain information about you. When using our website, we use cookies which allows the website to function, collect useful information about its visitors, and help us improve user experience. For more information, see our cookie policy.

When reserving an appointment on our homepage, we ask for your name, telephone number, and e-mail address. We use this information to confirm your treatment, as well as provide you with relevant information regarding services we provide. If you prefer us not to contact you in the future, we save a copy of your request and include this request in your journal, complying with your request. We use external data processors and data controllers who have access to personal data from our website and our software systems to fulfill the purposes, which is, among other things, used for bookkeeping and for backup purposes. The data processors handle personal data according to our instructions and only for the purposes we have mentioned here in this privacy policy document. These companies are subject to various security requirements, so that your data is treated confidentially and properly. Other companies responsible for data only receive personal data from us on a legal basis and are independently obliged to comply with applicable legislation.

We will not send your personal data to countries outside of EU/EEA. Should this become necessary, it will be done in accordance with the legislation, including securing the safe transfer of such data.

We regularly review our policy regarding the protection of personal data and update it, as our services and the use of personal data evolve. If we wish to use your data in a way that we have not specified we will contact you, and ask for your consent.

Every time we update this document, we assign a new version number.
